JotForm or other collect form?

Has anyone had success with using JotForm or some other data collection method for their blogs? I need a form that collects to a data source (txt, csv, xls as opposed to emailing data to me) and can be downloaded.

Just to answer my own question for posterity purposes, I think that JotForm rocks so far. I really easily built a form and am using it for Eat Local Challenge sign-ups. It collects to a database (as opposed to just emailing you the sign-ups) and has some cool functionality. It's $0/month if you have <100 form submissions and $9/month if you need more.

I had to jigger with the source code a tiny bit in order to make it fit the blog width, but other than that it's been pretty plug-and-play.

So far, it seems great and my data is coming in very nicely. You can see it in action here:
